IV errors investigated


Intravenous products: NursingTimes.net report “Senior nurses could form special investigation teams which would be dispatched to English hospitals after a “never event. The planned teams are modelled on the “go teams” used by the aviation industry to investigate disasters or near misses with the aim of establishing precisely what went wrong. In the NHS, they would be called in to investigate lethal never events such as patients being administrated chemotherapy drugs through the wrong method or the intravenous administration of undiluted potassium chloride.

Nursing Times has been told the National Patient Safety Agency, the Care Quality Commission and patient safety charity the Clinical Human factors Group are in talks with the Department of Health to set up the special teams as part of a patient safety initiative.”
