Abstract:

The disinfection of injection ports first appeared in the CDC Guidelines for Prevention of Intravascular Catheter Infection in 1982, however compliance to the recommendation has been since been poor. Colonization of the internal lumen of catheter hubs is the most significant risk factor for CRBSI within one week of insertion. The goal of this presentation is to review existing science and procedure recommendations for access site and catheter hub/stopcock disinfection.

Abstract:

This program is intended for the bedside care provider and those interested in best practices for the prevention of Central line associated bacteremia ( CLABSI). Prevention measures described will include evidence-based activities that decrease or eliminate the risk of infection.  Current literature sources further endorse and support recommendations made by the CDC in 2002 and will be described in this presentation. The Infusion Nurse’s Society (INS) standards will be described as they relate to prevention activities and best practices in the care of the patient with invasive vascular devices. The APIC program,  Eliminating catheter related complications, A Guide for More Effective  Infection Prevention and Control,  will be also be highlighted as a prevention tool for bedside healthcare providers.  Limited discussion will include engagement and empowerment of bedside care staff, utilizing teamwork to prevent and control  infections associated with these invasive devices.

On September 24 2009, Sophie Harnage RN, BSN Clinical Manager Infusion Services, Sutter Roseville Medical Center presents ‘Strategies to Prevent Catheter Related Bloodstream Infection’

Webinar Description:

Catheter Related Bloodstream Infection (CRBSI) is a major cause of patient morbidity, mortality, and cost. Lower CRBSI rates would decrease inpatient length of stay, and improve patient safety and satisfaction. The importance of reducing CRBSI cannot be overstated. An innovative central line bundle was developed at Sutter Roseville Medical Center to reduce CRBSI. Bundle selection was based on available research, CDC and IHI recommendations, new product technology, the amount of change required by the bedside nurse, and product ease of use. An innovative combination of focused nursing practice and product technologies were selected for the bundle and implemented through a defined educational program. To achieve success with limited personnel resources, a combination of behavioral practices and product technologies associated with lowering CRBSI that supported existing nursing practice were selected. When prevention practice activities and innovative products are combined the synergist effect is greater than when the same strategies and or products are used independently. Having a specialized team at Sutter Roseville Medical Center  has resulted in a Zero CRBSI rate in over 7000 PICC lines inserted and maintained by this team, for the past 40 months. While some of the bundle components have not been widely researched, the early outcome of this bundle provides a basis for additional study and refinement.

Intravenous conference: SCOTVAN – Call for papers - Scottish Vascular Access Network First Annual Conference, Thursday 12th and Friday 13th November 2009. The venue is The Beardmore Hotel and Conference Centre, Clydebank, Glasgow.

SCOTVAN are pleased to invite abstracts for Presentation and Poster Exhibition at their Inaugural 2 day Conference.The conference will be the beginning of a National Vascular Access Network establishing a collaborative group which will develop credibility by terminating the unknowns, discrepancies and misunderstandings surrounding Vascular Access in Scotland. This will ultimately benefit patients and health-care professionals across the country. Our aim is to promote sharing best practice and provide a platform for discussion and debate. 

Guidance for Submission: 

  • Submissions  should be summarised in no more than 250 words 
  • Authors should indicate their preference for Poster or Oral Presentation 
  • Submissions  should not contain diagrams or references 
  • All places, organisations and people  should anonymised 

Conditions of Submission: 

  1. The presenter author must register to attend the conference prior to submitting an abstract(s). Payment is not required until confirmation of acceptance. 
  2. Abstracts will only be accepted if received online in the requested format by Friday 21st August 2009. An email confirmation of receipt will be sent within 48 hours. 
  3. The first Author will receive all correspondence. 
  4. If selected for Oral or Poster presentation, authors are required to make their presentation available as a pdf file for posting on the SCOTVAN conference webpage. 
  5. The first author will be notified of the decision of the selection panel via email by Friday 18th September 2009. The Panel’s decision is final. 
  6. Accepted abstracts will be reproduced in the printed and online programme.

The Liverpool PCT home IV service provides gold standard Intravenous Therapies to patients in the community whether it is in their own homes or in community NHS clnics thereby reducing the need for hospitalisation and increasing patient ability to return to their preferred place of care. 

 Alison sates “We are commissioned to provide IV antibiotics, Blood Product Transfusion, Intravenous Iron, Care and maintanence of Vascular accesss Devices and insertion of Midlines plus IV support for patients with long term conditions and Bronchiectasis”. Alison continues “The service provides clinical leadership, education, assessment and support to the PCT District Nursing and Community Matrons and partnership working with GPs and secondary care in the management of patients requiring intravenous therapy”.

Vascular access team business case

Deborah writes “This year there has been a huge rise in the number of people contacting me requesting a copy of a business case that I completed in order to expand my IV Therapy Team in Brighton”.

Deborah continues ‘Increasingly business cases are required when attempting to introduce anything new into organisations, whether it is a new member of staff, a change to practice or a new piece of kit. No one taught me how to do this, so my first effort took almost 2 years to complete! To avoid others having to go through this trauma, I have written and article and will be presenting ‘How to put a successful business case together’ This will include who you need to talk to, how to overcome the multitude of barriers easily and quickly and provide people with a simple template that can be used again and again”.

Stephen Rowley on ANTT

Intravenous conference: Stephen Rowley, Lead Cancer Nurse UCLH and ANTT UK Lead is presenting at the IPS IV Conference in Harrogate. Stephen will be talking about implementing ANTT as an NHS Trust standard aseptic techniqe. Stephen explains “Since 2004, 76 of 175 Acute NHS Trusts in England have requested the ANTT implementation pack. A review of hospital data and experiences following ANTT implementation demonstrates ANTT is an effective framework for standardising and improving aseptic practice across large clinical workforces. Findings also suggest this can have a positve effect on helping reduce hospital acquired infection. The reasons why ANTT has been so widely adopted and is the NHS’s first standard defined aseptic technique are discussed. Future plans for the project will also be given”.

Intravenous history at IV conference

Intravenous conference: Dr Neil MacGillivray, Honorary Post Doctoral Fellow in Scottish History University of Edinburgh will present the inaugural Dr Thomas Latta Lecture for the Intravenous (IV) Therapy Programme at Infection Prevention 09.

While focussing on the pioneering work of Dr Thomas Latta the paper will also examine the social, political and medical upheaval which resulted from the arrival of cholera in Europe in 1831: medical men were assassinated in Russia and there were real fears of revolution in many European countries.

The response of the medical profession will be reviewed referring to the work of Dr William Brooke O’Shaughnessy and examining the general helplessness of the profession in the face of this new disease when the mainstay of treatment was bloodletting. Not only did the new disease result in the first cardiac catheterisation but also the first use of intravenous saline by Dr Thomas Latta.

Safe practices webinar

Intravenous conference: To guide healthcare systems in providing care that is free from error and harm the National Quality Forum (NQF) is releasing its 2009 Safe Practices for Better Healthcare.  To accelerate the pace of adoption of Safe Practices nationwide, a year-long Webinar series will begin in April to provide Safe Practices implementation strategies and commentary from experts in the field.

“The Safe Practices offer clear tools for those who provide, purchase, and use healthcare to ensure that harm is reduced and care is safe.  While improvements have been made in patient safety, they must spread farther and faster,” said Janet Corrigan, NQF president and CEO. “We cannot afford — in lives or dollars — to provide care that is unsafe. Every patient deserves safe, high-quality healthcare, every time they receive care.”

Preventable errors cost the United States an estimated 98,000 lives annually, $17 billion to $29 billion per year in healthcare expenses, lost worker productivity, lost income, and disability. Even more troubling, while healthcare spending grows more than seven percent per year, it is estimated that patient safety is improving by only 1 percent or 2 percent.

The IPS have announced the details of the IV Conference that is running in conjunction with the Infection Prevention 09 Conference.

The new IV Forum will be presenting their first full day’s programme in conjunction with the main IPS conference programme on Tuesday 22nd September.  This session is aimed primarily at people working in IV therapy but it will also be of interest to anyone working in infection prevention and control. Delegates can either register for the IV track on its own or register as a delegate to the whole IPS conference. Any delegate can select which sessions they attend on each day from the three main tracks in the programme.

The programme includes:

This national programme in April has been jointly developed by the Infection Prevention Society (IPS) and the EPIC2 authors/Thames Valley University. Infection prevention seminar

Intravenous conference: The Northern and Southern New Jersey chapters of the Association for Professionals in Infection Control and Epidemiology (APIC) will host a “Principles of Infection Prevention and Control” seminar March 30 through April 3 at the Clarion Hotel in Princeton, N.J. Free healthcare safety seminar

Intravenous conference: ECRI Institute, an independent nonprofit that researches the best approaches to improving patient care, is offering a free educational breakfast seminar, “Hazard and Recall Management: Best Practices,” which will be held at ECRI Institute’s Headquarters in Plymouth Meeting, PA, on Wednesday, January 21, 2009.

The seminar is designed for hospital-based biomedical and clinical engineers, materials managers, risk managers, patient safety officers, and other health professionals responsible for responding to product alerts. The program is aimed at providing insight into identifying and locating affected healthcare products and putting an effective alert management process in place. 

“Managing hundreds of device recalls can be a daunting task for hospitals,” says James P. Keller, Jr., M.S., vice president, health technology evaluation and safety, ECRI Institute. “Through this seminar, participants will learn about ways to efficiently manage this process so that critical information about potentially dangerous products does not fall through the cracks.”

Seminar attendees will also learn the common missteps in the hazard and recall process, as well as how to establish executive-level buy-in to the alert process. In addition, the seminar will feature a panel/audience discussion on the five most common challenges of hazard and recall management.

Symposium includes needlestick

The National Institute for Occupational Safety and Health (NIOSH), through the Division of Safety Research, has a broad research program focused on preventing traumatic occupational injuries. As part of this effort, NIOSH is hosting the fourth National Occupational Injury Research Symposium (NOIRS) on October 21–23, 2008, at the Sheraton Station Square in Pittsburgh, Pennsylvania. NOIRS is the only national forum for presenting research methods, data, and findings focused exclusively on the prevention of occupational injuries. Abstracts from the oral and poster session presentations will be posted to the NIOSH website after the symposium.

Leading researchers in occupational injury prevention will present findings about many major categories of injury, including needlestick. Those unable to attend the conference can participate electronically by posting comments, research, and real-world examples on the NIOSH Science Blog.

Marketwatch.com have reported that improvements in the prevention of catheter-related infections will be the prime focus at the annual conference of the Association for Vascular Access (AVA) September 11-14 in Savannah, Ga.

The conference comes on the eve of major changes in Medicare that will eliminate payments to hospitals that fail to prevent certain hospital-acquired infections.

Among conference presentations on preventing catheter-related bloodstream infections:

  • Allan Morrison, Jr., M.D., will describe the vision of an infection control program that can improve clinical outcomes and decrease infection rates, high-risk antibiotic use, hospital lengths of stay, and mortality.
  • Stephen Ash, M.D., FACP, and Deb Richardson, R.N., M.S., CNS, will review current data about the effect of catheter locks or coatings on the infection rates. The speakers will also examine the potential for these technologies to significantly reduce catheter-related infections in the future.
  • Kari Love, R.N., B.S., will present a poster to show how use of a closed luer access, split-septum device has eliminated catheter-related bloodstream infections for eight months in an adult medical-ICU population. The device replaced a positive-pressure mechanical valve.

MADRI conference on CRBSI

The Multidisciplinary Alliance Against Device-Related Infections (MADRI) hosted its eighth annual conference June 6-8, 2008 in Lansdowne, VA where leading infection specialists presented on a variety of topics, including research goals in epidemiology and infection control, legal perspectives on MRSA and catheter-related blood stream infections (CRBSIs).

“Driven by discussions around improving patient care and the prevention of device-related infections, the eighth annual MADRI conference was a huge success,” said Rabih Darouiche, M.D., VA distinguished service professor, Departments of Medicine (Infectious Disease Section) and Physical Medicine and Rehabilitation and director of Center for Prostheses Infection and MADRI founder. “In an open and educational atmosphere, experts across disciplines were able to discuss challenges associated with device-related infections and exchange ideas about prevention and improvement of patient care.”

One of the main themes of the conference was CRBSIs, a common nosocomial infection that develops when bacteria enter the blood stream through a central venous catheter (CVC).

Techniques for declotting VAD’s

On the third day of the 35th INS conference in Phoenix, Arizona, Jennifer Estela-Stollwerk, BSN, CRNI provided us with an extensive overview of techniques used for the declotting of venous devices. Jennifer described how 42% of catheter occlusions may be attributed to non-thrombotic events such as lipid deposits. The remaining 58% are described as thrombotic.

Jennifer described the use of ethanol, hydrochloric acid and sodium bicarbonate in the treatment of a variety of non-thrombolytic occlusions.

For the treatment of thrombolytic occlusions, Jennifer describes both treatment with medication and mechanical methods of declotting.

Marcia Ryder on biofilm

Marcia Ryder PhD, MS, RN began her session at the 35th INS conference in Phoenix, Arizona with a stark reminder. She told the assemble audience that during her one hour presentation something in the region of 30 people will die in the US of a healthcare associated infection (HCAI)!

The audience are then informed of another stark reality associated with IV’s and the treatment of infection. Bacteria organise themselves into a cooperative community. This bacterial community is held together with biofilm.

Marcia held the attention of her audience for the full hour. The presentation included many informative messages, the two messages that I would like to share is that ‘…all HCAI’s are are a biofilm disease’ and ‘…health care professionals need to re-think the basic things that we do’. Finally, Marcia reminded us that 100 trillion bacteria reside on our skin!

INS 2008 kicks off in style

The 35th INS conference in Phoenix, Arizona commenced in style this morning at 8am with a keynote address from Tom Hayes who left the audience with tears in their eyes. Tom brought humour to the proceedings whilst delivering his message of live, love, laugh and learn.

This was followed by the first general session ‘models of care for vascular access teams’. 

The exhibition is supported by approximately one hundred companies, covering every aspect of IV therapy. products debuting at the conference include the new Chlorhexidine impregnated dressing from 3M and Baxter’s ‘V link silver impregnated IV connector’

AVA 22nd annual conference

The Association of Vascular Access are accepting abstracts for podium and poster presentations for their 22nd Annual Conference, September 11-14, 2008 in Savannah, GA.

Presentations must fit one of the following categories and bear a direct relevance to the field of vascular access – a) Clinical Research, b) Innovations in Education or Practice, c) Health Care Policy & Delivery or d) Clinical Case Study/Quality Improvement.

All abstract submissions must be submitted NO LATER THAN 12:00 Midnight PDT on June 16, 2008. Authors will be notified by email of acceptance as soon as possible following review.

3M study day was a blast

I recently spoke at the 3M Advanced IV Study day at the National Space Centre on the 18th October 2007.3M spacemanThe day was chaired with his usual professionalism by Martin Arrowsmith (Professional Service Manager, Skin Health, 3M Healthcare). First to present was Carmel Streater (Specialist Nurse Manager, Addenbrookes Hospital, Cambridge). Carmel described her central venous access service with a dedicated purpose built vascular access unit.Following coffee, Ann Reed (Solicitor, Medeagle UK) kept the whole audience riveted with her knowledge and experience of assisting claimants with negligence claims against the health service. After lunch it was my turn. I offered an overview of peripheral IV problems followed by a focus upon nursing actions that can assist in problem prevention.In particular I looked at the standards that can be achieved with a vascular access team. Once I had finished Hayley Reading (Modern Matron, Leicester Royal Infirmary) demonstrated how she adapted and introduced the PIVA tool to improve the standard of peripheral IV care.Following completion of the day. Some of us went on a visit around the National Space Centre. To conclude I would recommend attending one of the 3M Advanced IV Study Days and visiting the National Space Centre.
